Normally it is low budget film and they want to see how well it does in the test market before taking it everywhere. I can also be that a certain chain purchased the movie for showing.

Not enough money for distribution.
Maybe there small budgeted. Or they don't think there going to be a big hit. Or maybe the companies didn't want to show that movie, if they thought it wasn't going to bring in any money.
could be a controversial film that people dont want to see in that region, could be a sure dead winner and not make any money for the theater so they decline screening varies why was the al gore film knocked down to only the small indie theaters and no movieplexes -controversy and people had doubts on its cash value though i'm sure it took in a good amount of change
They know that these movies are not five stars .
Mostly movies of low budget .
But sometimes they break this . An example : " A Walk To remeber " .
It's usually the theatre's fault. They refuse to screen films which they think won't make profit. When 'Star Wars' was first released few theatres wanted to show it, the studio forced them to do it by including it on a package with a highly anticipated movie (which nobody remembers now)
